Collaborative, team-based approach to support youth with complex behavioral health needs.
High Fidelity Wraparound Services (HFW) is a community-based, team-oriented process designed for children and youth ages 3-20 with complex behavioral health challenges. These youth may have involvement with multiple systems, such as mental health services, child welfare, or juvenile justice. The service involves developing individualized care plans through collaboration among youth, families, service providers, system partners, and natural supports.
HFW is led by a trained facilitator who guides a team through all stages of the process, from assessment and goal setting to plan implementation. The team works closely to coordinate services and ensure the youth and family receive the necessary supports. Care plans typically last up to 18 months and aim to strengthen the family’s ability to manage services and support their child’s needs effectively.
HFW is important because it provides a holistic approach to supporting youth with complex needs. By working together across different services, it helps avoid gaps in care and makes sure families have the information and support they need to make choices and get the right help.
